Name in dedication

My name is completely different now than what I was born with. Do I use my birth first middle last name or my legal first middle last name for the dedication?

You use whatever reflects who you are and makes you happiest. You label yourself who and what you wish to be labeled as, be who you are, it is only through being yourself that Satan will accept you. Just use the name that you feel in your heart, mind and soul is a true reflection of who you are, otherwise you can make up a whole new name altogether that only the Gods of Elysium will refer to you as.

Hmmm... Well my birth name has always been my name... So I guess that's what I'll go by... I should use my suffix right? Probably I would imagine...

Like I said, you just go by what you feel reflects who you are and what makes you most comfortable. Use whatever you want, as long as it makes you feel like yourself
