What time does the moon sign change? Does the moon sign change at 12am or at night when the moon first comes up?

Neither. The moon sign change varies and can be found on an ephemerides where the moon enters the next sign.

The moon, when visible is often visible during the day as well, it is just much lower in the sky.

The moon sign change is different each days and occurs every 2 days or so.
